Insurance

Northwest Vision participates with many of the major insurance companies. We are direct providers for the following insurances:

Anthem
Cigna (PPO and OAP)
Flora Network
Medical Mutual of Ohio
Medicare
United Health Care
Vision Benefits of America (VBA)
Vision Plus
Vision Service Plan (VSP)

If you have an insurance plan other than the one listed above, payment is due in full at the time of service. We will be happy to provide you with copies of your receipts so that you may submit for reimbursement from your insurance company.

It is important to bring your insurance cards and referrals (if required). We will be happy to review your coverage with you and assist you by completing and submitting all your insurance forms promptly. Please remember that even with insurance, most plans do not cover fees in full. You will be responsible for any co-payments, deductibles, or non-covered items.
